To assist in direct resident care under the supervision of the Licensed Nurse and to assist in maintaining a positive and psychosocial environment for the residents.
Essential Functions:
- Assist residents with direct resident care.
- Perform or assist with ADL’s, using care plans and flow sheets as a guide in the following.
i. Bathing and personal hygiene

- Dispense prescribed medication orally to resident per physician order
- Verify resident medication schedule and document all administration accurately
- Ensure right resident is getting the right medication, at the right dose, through the right route and at the right time.
- Report unusual responses to medication immediately to the nurse
- Administer creams, ointments, and patches as prescribed
- Safely assist residents in using inhalers or nasal sprays
- Use proper tools for accurate dosing of syrups or solutions
- Coordinate medication timing around meals as required.
- Prevent cross-contamination during medication handling
- Log administered medications, refusals or missed doses.

General Summary of Duties:
Physical Therapist Assistant(PTA) work under the supervision of a licensed therapist physical therapist to help the patient improve mobility, strength and function. PTAs carry out treatment plans by guiding patient through therapeutic exercises and activities. PTAs observe and report patent responses to therapy ensuring treatment are safe and effective. PTAs assist with patient education, documentation, and use of assistive devices. The goal of PTAs is to support recovery and help patient regain independence in daily activities.
Essential Functions:
Patient Care and Treatment
- Implement physical therapy treatment plans developed by a licensed Physical Therapist
- Instruct patients in therapeutic exercises to improve strength, flexibility, and endurance
- Assist patient with balance and gait training activities
- Apply physical therapy modalities like ultrasound, electrical stimulation, heat/cold therapy
- Guide patient in functional mobility training like transfers, bed mobility, ambulation
- Assist patient in the proper use of assistive devices like walker, canes, crutches and wheelchairs
- Observe patient during treatment and monitor their physical responses and safety

Collaboration & Teamwork
- Collaborate with PTs, nurses, occupational therapist, speech therapist and physicians.
- Attend interdisciplinary team meetings and case conferences when required.

- Set up and clean treatment areas and therapy equipment
- Ensure equipment is functioning safely and report any malfunctions

- Investigate and report incidents, accidents and adverse events.
Administrative & Operational Duties
- Manage the nursing department budget, staffing levels, and resource utilization.
- Participate in strategic planning and contribute to overall facility leadership
- Coordinate with scheduling and Human Resources(HR) to ensure proper shift coverage and staffing ratios.
- Ensure timely reporting and follow up on resident and family concerns.
- Maintain accurate records and reports for nursing operations and compliance.
Collaboration & Communication.
- Serve as a Iason between nursing, medical, administrative, and support departments.
- Facilitate care coordination with physicians, therapists, and social workers

Qualifications:
- Required: Must be 18 years of age and hold an Associate of Applied Science Degree in Nursing degree(AAS) or related field, a registered nurse with a valid and unencumbered Maryland State RN license with at least six years’ experience including four years of administrative or supervisory responsibilities.
- Preferred: Bachelor’s degree in nursing (BSN), a registered nurse with a valid and unencumbered Maryland State RN license with at least four years of RN experience including two years in supervisory, teaching, consultative or administrative capacity relevant to the DON’s role.
- Recommended: Master of Science in Nursing(MSN), a Doctorate degree in nursing or related field, a registered nurse with a valid and unencumbered Maryland State RN license with more than two years’ experience including teaching, consultative, administrative or supervisory responsibilities.
- Must pass state and national criminal background check as mandated by health occupations. Must demonstrate good moral character well beyond passing criminal background check or clean professional disciplinary history.
- Must demonstrate competency in written and spoken English language, strong emotional intelligence and communication skills. Excellent organizational ability, good time management and problem-solving skills.

General Summary of Duties:
The facility administrator is responsible for overseeing the entire operation of the facility not just the clinical operations. The facility administrator is charged with ensuring regulatory compliance, financial performance, quality resident care and management of all departments of the facility operation. The administrator manages staff across all departments, coordinate with medical and nursing leaderships, and ensures the facility meets state and federal requirements. Essential Functions:
Operational Management
- Oversee the daily operations of the entire facility to ensure smooth and efficient functioning.
- Develop and implement facility policies, procedures and standards

- Coordinate interdisciplinary departments including medical nursing, therapy, dietary, housekeeping and administration.
